How to Prepare for your Telemedicine Visit
*Allow yourself 20 minutes to prepare for your visit!
*Please have the following available at the visit:
-Height
-Weight
-BP (if available)
-Pulse, Temperature
-PF-Peak Flow Score (best of three) If on asthma inhalers do daily pre and post rescue inhaler
-Flashlight or smartphone light
-Your Pharmacy Zip Code
For breathing difficulty/Asthma Patients only:
-ACT Asthma Control Test Score (download below) Do daily scores if on daily asthma inhalers/nebulizers
For reflux, heartburn or throat clearing:
- RSI Reflux Symptom Index (download below)
For Sublingual Immunotherapy (SLIT):
-EpiPen or generic alternative at hand with instructions for review
-SLIT tablets at hand with instructions for review
What To Expect
We review your detailed history followed by a virtual 'physical' examination. We review your peak flow scores, asthma control test scores and reflux symptoms index scores if indicated and discuss treatment options, prescriptions, and a management plan in the safety and convenience of your home or workplace.
Procedures
Environmental or Food or Medicine Allergy
Lab Requisition for Blood test for RAST or RadioAllergoSorbentTest of allergens is provided or allergy skin tests are recommended.
Allergen Immunotherapy (SLIT- SubLingual ImmunoTherapy)
First Dose: Via TeleMedicine with instructions and EpiPen present.
Tablet is placed under the tongue. After the first dose in doctor’s supervision, tablet is taken at home daily.
GRASTEK® and ORALAIR® for pasture grasses such as Timothy taken before and during the plant-growing season. GRASTEK® can also be taken year‐round for 3 years.
RAGWITEK® for ragweed taken before and during the plant‐growing season.
ODACTRA™ for house dust mite allergy is given year‐round.
Onset of improvement is during the first pollen season!
*All patients have an epinephrine auto‐injector in case of a severe allergic reaction.
Peak Flow Meter or Smart Peak Flow Meter with App
Use daily when on asthma inhalers.
Post Procedure
SubLingual Immunotherapy
Wait for 20 minutes after taking your sublingual tablet before eating or drinking. In the rare event you experience systemic symptoms go to nearest emergency room or call 911 (use EpiPen for anaphylactic emergency)

*Important: If you have been prescribed an EPIPEN or ProAir (Albuterol) rescue inhaler please keep it with you at all times!

Your telemedicine visit is likely covered by your insurance company so please verify. You may contact Danielle at dbailey@sticomputer.com or Diane at evansdiane649@gmail.com for billing queries. You are responsible for copays, deductibles, and ensuring referrals from your primary care physician prior to your visit. For patients without insurance coverage, payment is expected at time of service via PayPal or Doxy.Me by credit card.
